How it works

A plan for your first days home

Research shows the days right after discharge are when confusion about medications and warning signs sends people back to the hospital. Each package is built around the same simple structure.

Step 1

Request your package

Choose a 3, 4, or 5-day package and request a time through the secure SimplePractice portal. Intake forms and payment are all handled there.

Step 2

Day 1: your intake call

A 60–75 minute phone call to go over your discharge instructions together — what the hospitalization was about, medication additions and changes, follow-up plans and appointments, and the red-flag issues that mean you should seek care.

Step 3

Days 2 and beyond

For the rest of your package, you have open text messaging with me — and phone calls if necessary. Questions about a new pill, a symptom, or a follow-up visit don't have to wait.

I'm Monica Helton, a board-certified family and emergency nurse practitioner. I've spent my career in emergency rooms and urgent care, and I've seen the same story again and again: patients go home with a stack of discharge papers, new prescriptions, and no one to call when the questions start.

I started this practice to be that person. I help patients and families navigate life after a hospital discharge with expert guidance, education, and support — compassionate guidance and peace of mind, right when you need it most.
